At the beginning of first class you’ll be asked to sign a printed copy of this form to confirm all the information is correct.

Liability Waiver: By submitting this form we acknowledge we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this registration form / release form in person at the commencement of our first class at TorontoDanceSpotTM, we (the dancers) assume all risks related to the use of any and all spaces used by TorontoDanceSpotTM.
We agree to release and hold harmless TorontoDanceSpotTM, including its teachers, dancers, staff members and facilities used by TorontoDanceSpotTM from any cause of action, claims, or demands now and in the future. We will not hold TorontoDanceSpotTM liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, we agree to obey the class and facility rules and take full responsibility for our behavior in addition to any damage we may cause to the facilities utilized by TorontoDanceSpotTM.
In the event that we should observe any unsafe conduct or conditions before, during or after our classes, we agree to report the unsafe conduct or conditions to the Executive Director, Artistic Director, Individual Instructor or any staff member as soon as possible. We have familiarized ourselves with attendance and cancelation policy available on the TorontoDanceSpotTM web site - we understand group packages are for consecutive weeks and there are no refunds.
